This appointment type is when a GP Management Plan is in place. A GP plan allows for an organised approach to your care and allows referrals to allied health services of which diabetes education and dietitian service falls. A referral for diabetes education (Item 10951) or dietitian (Item 10954) will allow a Medicare rebate. To claim the Medicare rebate a GP referral will need to be provided to HealthMed Lifestyle Clinic.
