NSW Firearms Risk Assessment requires the following to be provided: - Letter from the registry - Risk assessment questionnaire - Signed consent for release of information - GP Referral We will review this information prior to confirming the appointment to ensure we are able to provide an appropriate assessment. Full payment is required before release of the report to the registry. We will post directly to the registry as outlined in the letter they have provided you.
