This appointment type if for new clients and existing MHCP clients. If you are a new client you are required to complete the intake form. If you are an ongoing client disregard the intake form. Please note: full upfront payment is required to confirm your initial appointment. This is a 90 minute appointment, and is hourly charge (starting at $140ph) varies depending on practitioner.
